Additional Information


Responsible for indirect patient care in the UT Health Austin Access Center. Provide direction to Medical Assistants, as well as other non-licensed personnel in the provision of patient care primarily telephonically and electronically.

Essential Functions

Provides individualized patient coaching focusing on training and encouraging patients into a state of wellness. Identifies patient personal strengths in an effort to recruit them to participate in improving and maintaining their health. Performs patient assessments/reassessments. Performs patient triage in urgent/emergent situations. Documents all care coordination contact interactions with patient and providers. Schedules patient appointments. Teaches, supervises, and services as a clinical resource person and delegates in accordance with the competency level of all staff members. Serves as a mentor and completes the appropriate documentation of training and competency. Receives and processes specialty care referrals from providers ensuring timely coordination of care for patients. Documents all referral processing steps throughout multiple stages, care coordination interactions with patients and providers. Provides patient with post-procedure education and support. Interprets reports, data and medical information to determine the needs of the patient/family. Maintains patient care standards. Participates in organization focus groups. Performs a variety of outbound proactive health contacts to patients, using established electronic methods. Provides patient with post visit education and ensures patients receive follow up care as appropriate. Performs prescription refills and provides medication management teaching. Provides lab result information to patients. Complete patient medication prior authorizations as necessary/when applicable. Establishes and maintains effective working relationships with care teams internally and externally. Navigates all contacts appropriately using various software tools and electronic technology methods. Maintains quality performance review scorecard standards (score of 90 percent or greater). In collaboration with other health team members, follows the philosophy, policies and goals of UT Health Austin.

Required qualifications

Registration with the Texas State Board of Nurse Examiners. Graduation from an approved school of nursing. Three years as a licensed registered nurse. BLS certification. Bilingual in English and Spanish. Proficient use of computers. Working knowledge of Electronic Practice Management systems to include scheduling and general billing function. Capable of documenting patient communication, referral notations, and clinical chart notes accurately and appropriately. Professional verbal and written communication. Ability to anticipate needs and follow through with internal and external customers. Exceptional customer service skills, awareness and phone etiquette. Ability to follow directions. Ability to communicate effectively with patient and healthcare team, including the ability to explain registration policies and procedures. Ability to use experience and judgement to plan and accomplish goals. Ability to function in a fast-paced environment. Good organization, problem solving and critical thinking skills. Knowledge of operating a multi-phone system or electronic telephone software system. Ability to adapt to a rapidly changing work environment. Equivalent combination of relevant education and experience may be substituted as appropriate.

Experience working in a multi-practice setting. Experience with occupational health. Experience with scheduling surgeries. Experience with scheduling diagnostic imaging. Experience with obtaining insurance authorizations.
